Please follow all the instructions. We will require the Applicant to send FORMS 2, 3, 6 ONLY; and FORMS 4 and 5 should be completed and sent to us by the people/references you have chosen; however, in the case that they are not able to, and if they completely trust that you will not read their answers, you are allowed to send in all FORMS (#2, #3, #4, #5, and #6).

ONLY THE APPLICANT should fill, answer, complete these forms:

STAFF APPLICATION – FORM 1 “Staff Application Form Guidelines” (1 Page)

STAFF APPLICATION – FORM 2 “Actual Staff Application Form” (5 Pages)

STAFF APPLICATION – FORM 6 “Commitment Form” (1 Page)

ONLY THE APPROPRIATE PERSON should fill, answer, complete these forms:

STAFF APPLICATION – FORM 3 “Health Form” (2 Pages)

STAFF APPLICATION – FORM 4 “Pastor’s Reference” (3 Pages)

STAFF APPLICATION – FORM 5 “Employer/Leader/Teacher’s Reference” (3 Pages)

*Please NOTE that in order to join YWAM Juba (or any other Bases as Staff), you are required to have completed a Discipleship Training School from any YWAM base across the globe. Submitting a copy of your DTS Certificate will also make it easier for us when we do checks. Thank you for understanding.
